Impact Jersey ‘CareTech’ grant creates more time to care for FNHC

Digital Jersey is delighted to announce a further grant through its Impact Jersey ‘CareTech Challenge’ Programme.

The grant will support Family Nursing and Home Care (FNHC) to deliver a digital monitoring system that will help to support frailty and the incidence of falls in year one, and in year two expand to complications with wound care and diabetes management, that often lead to hospital admissions, particularly amongst the elderly population in Jersey.

Through the funding, which totals almost £795,000 over two years, the pilot project will see FNHC partner with Graphnet Health to implement the remote monitoring technology platform ‘Luscii’. A range of healthcare devices will empower patients and their clinical support team to track key health data and transform community healthcare delivery in Jersey, particularly for the elderly and those with frailty and long-term conditions.

Over the two-year period, Luscii will gather data for around 1,000 patients across the community in Jersey to reduce the number of falls, enhance the management of diabetes and improve the monitoring of chronic wounds. In turn, this will serve to reduce complications, identify potential issues before they arise, and ultimately alleviate pressure on the emergency services and hospital resources.  The pilot project has the support of the Government healthcare service and is aligned with the digital health strategy for Jersey.

The roll-out of Luscii will also aim to improve the quality of life and independence of older adults in Jersey, whilst also increasing healthcare provider efficiency and enable better care coordination.

FNHC is a Jersey charity focused on helping islanders to start well, live well and age well. It provides expert prenatal and parental care and supports children, young people, families and members of the community who need healthcare or support in their homes.

FNHC’s Luscii project joins a number of other projects to have received grants through the CareTech Challenge earlier this year, including Orchid Connect, Good Boost and Lifestyle Medicine.

Funded by the Government of Jersey and delivered by Digital Jersey, the ‘CareTech Challenge’ was launched to offer a total of up to £2m in grant funding to innovators who could develop solutions aimed at improving the lives of older people and delivering long-term, high-quality care in Jersey through the smart use of technology.

It aligns well with Jersey’s future, increasingly digital, health care strategy. And perhaps most importantly, it will improve health outcomes for our friends, neighbours and family members and enable FNHC to reach more islanders with targeted and preventative care that will impact the lives of those we love in our community.”
